We are seeking a compassionate and skilled Nurse Practitioner to deliver comprehensive primary care services to residents in assisted living facilities. This role is ideal for someone passionate about making a difference in the lives of the geriatric population. The Nurse Practitioner will work collaboratively with an interdisciplinary team to provide high-quality, patient-centered care, managing both chronic and acute conditions while promoting health maintenance and disease prevention.

Responsibilities

  • Conduct comprehensive health assessments, including physical examinations, medical histories, and medication reconciliation.
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team, tailored to each resident’s unique needs and preferences.
  • Manage chronic conditions and acute illnesses, providing timely interventions and appropriate referrals as needed.
  • Order and interpret diagnostic tests, such as laboratory tests and imaging studies, to support diagnosis and treatment planning.
  • Prescribe medications and treatments within the scope of practice, adhering to state regulations and facility policies.
  • Participate in care initiation for chronic care management services.
  • Provide education and counseling to residents and their families on health maintenance, disease prevention, and self-care strategies.
  • Engage in quality improvement initiatives and interdisciplinary meetings to enhance care quality and resident outcomes.
  • Maintain accurate and up-to-date documentation of all assessments, interventions, and communications in the electronic medical record system.

Qualifications

  • Master’s degree in Nursing and graduate of an accredited Nurse Practitioner program.
  • Unrestricted New York State Nurse Practitioner license.
  • Unrestricted Drug Enforcement Administration (DEA) permit to prescribe all medications commonly prescribed in the course of practice.
  • Eligible to provide reimbursable services and maintain provider status with Medicare and Medicaid.
  • A minimum of 3 years of clinical experience, preferably in long-term care (preferred).
  • Experience with eClinicalWorks (eCW) electronic health records (preferred).

Compensation and Benefits

  • Competitive salary ranging from $60,000 - $120,000 based on experience and hours worked.
  • Sign-on bonus for direct hires:
    • $2,500 at hire
    • $2,500 at 90 days
    • $5,000 at one year, totaling $10,000.
